What power converter should the AdderView 4 Pro DVI Dual-Screen include?

I have had mine in use for less than two weeks and the included power converter blew out (reads zero volts at the tip now). Marked on the power converter it says 5V 4A, but the KVM switch says it wants 12V 5A.

Are you referring to the power adapter that ships with the switch?[IMG]
